Annual Wellness Visits are free preventative health care exams for our Medicare recipients, where your provider looks for signs of potential health risks. These visits are not physical exams.
Scheduling – these visits are comprehensive and require an hour. Since these are not physical exams, we schedule them as telehealth appointments to be completed in the comfort of your home. In-office visits are available too.
Screenings – After scheduled and prior to your appointment date, we will send you an assessment to jump start the virtual visit. This assessment is in the form of a questionnaire and will be reviewed during the appointment.
Since this is not a physical exam, the first portion of the visit, about 30 to 40 minutes, our medical assistant will begin the virtual visit and review your answers with you.
• Vitals and measurements
• Cognitive impairment
• Fall risks
• Vision and hearing
• Review of your current medications
• Recent hospitalizations
• Review of your home environment
You will then be transferred to your provider to complete the appointment.
Physician Visit – The physician will join the last portion of the call to discuss your wellness goals and any preventative services that are needed. Also, the physician will answer any questions you may have.
Schedule follow-up – The next step may be for you to have a follow-up appointment to address any chronic diseases or concerns identified by your care team. This can be scheduled at the conclusion of the visit.

The AWV appointment is only a virtual visit, being in the office is not a requirement.
This is more of a preventative health assessment and wellness visit. Your provider will review a list of questions that are geared towards disease prevention and your personal healthcare goals.
The questionnaire will be sent to you prior to the virtual appointment, to allow more time to address your healthcare needs and review your responses.
No lab work and physical contact for the AWV unlike the annual Physical exam.
The earlier we schedule the more time throughout the year you will have to address and treat any possible health issues or new care plans for the future.
Completing the assessment prior to your appointment will allow more time for your actual appointment.
You will be given personal time with your provider to discuss your health concerns, progression and given information on how to build a healthy lifestyle for the year.
The visit is for our Medicare recipients and will not cost you anything. It is absolutely free!
A virtual call from your healthcare team first; with an AllCare Georgia Medical Assistant and then with your Provider.
A risk assessment that is geared towards wellness for our Medicare patients. The questionnaire is designed to accurately review the patients’ health and wellness needs.
At the end of the assessment, your Provider will be able to create a plan of care that aides in governing your overall health for the year.
You’ll be provided with tools and local resources to assist you with your everyday living and health.
An option to review your social needs, such as your living environment, access to resources and family circumstances. Your provider will discuss how these social needs impact on your health and how we can help you to improve them.
Follow-up appointments and continued care can be scheduled at this time as well.
