Transitional Care Management

When you are discharged from a hospital or skilled nursing facility, AllCare Primary & Immediate Care will provide outreach and other healthcare services to aid your smoother transition. This is called TCM.

TCM services consist of but are not limited to:

  • 24–48 hour contact call from AllCare Primary & Immediate Care after discharge
  • Setting a follow-up appointment within the next 7- 14 days
  • Medication review and quality continued care services if needed.

Transistional care is crucial for you as our patient as it ensures a smooth and safe transfer from the hospital to your home.

  • That the time between discharge and your first outpatient appointment can be the most critical time for an adverse event (possible issues from the inpatient stay) to occur.
  • Your follow-up appointment can possibly be virtual.
  • You have 30 days from your date of discharge to be seen by an AllCare Primary & Immediate Care provider.
  • Allowing AllCare to assist you with transitioning back home, in most cases, will result in avoiding near future hospitalizations or inpatient stays.
  • Our team is here to provide you with a thorough review of your medications current and prescribed upon release. This medication reconciliation process avoids duplications, dosing errors, or omissions.
